That's always the problem with unknown hybrids, anything that brings together longifolium and caudatum in good measure without too much of anything else distinctive could look much like this. The older you can verify the plant to be the more likely it is to be Grande, an old and widely distributed cross, but it should always have a question mark next to any identification.
 
Phrag. Sorcerers Apprentice.... - thats always the problem with orchid specialists!
 
I agree with Tom. Seems Phrag Grande to me. Probably one made using longifolium as pod patent.

A big thanks to everyone who helped to identify the Phrag. After looking the possibilities, I think Phrag. Grande would be the best guess. After repotting the plant, I found remnants of a plant tag that was faded but could make out the word "Grande" on it!
