For what it is worth, I do what Leo does. I'll knock out as much old mix as I can, but mostly just pot up. The plants seem to figure it out pretty well.

Soap and Water!?! The knot is the old growth, I'd just knock out any bad [rrotted] media and put in a biggfer pot w/ new stuff poke into and aroung the root mass.
