I have no idea why, but Phrags seem to be particularly vulnerable to transient flower deformities. In my (limited) experience, when a flower comes out “odd” the other ones on the same spike are likely to do the same. So, now, if I see that, I cut the spike off and let the plant redirect its energy to a new growth.
On a positive note, my “avatar” (Phrag. Alfredo Manrique ‘Pacific Heights’) bloomed without a pouch the first time it bloomed for me. Each successive bud was the same. However, on the next bloom cycle, it was fully-formed and nice enough to get awarded.
