Paphiopedilum niveum (Perfect Circle x New Super Bowl). I purchased this plant from an orchid friend 10 years ago. Originally, this plant was from Orchid Inn (Sam). It blooms once every 2 seasons when a growth matures. Usually, this plant blooms in the summer, so it is about 4-6 months early this year (the fan started 2 seasons ago and I expected to bloom in the summer, not in the middle of the winter). Since the flower bud developed in a lower temp this time, I think the flower form is better than the last bloom. All the floral parts look more symmetrical this time.

Nice flower, but aren't its leaves too yellowish?
It's the camera from the natural light. The actual leaves are darker than the picture. The big fat yellow leaf in the middle of the pot is an old growth (bloomed growth), it's just in the process of dying back (normal).
